| 目的 探讨鹿角为君的温肾通络处方联合PKP手术对OVCF的临床治疗疗效。方法 研究对象为2022年1月至2022年12月期间在本院骨科接受PKP治疗的急性OVCF患者,采用单双号法将80名患者随机分为两组,分别为观察组(40名)及对照组(40名)。对照组联合PKP和抗骨质疏松治疗,观察组则在对照组的基础上联合服用鹿角为君温肾通络方。对比两组治疗1个月前后的总有效率、中医症候积分、VAS评分、ODI评分、JOA评分、血清中钙(Ca)、碱性磷酸酶(ALP)和25羟维生素Ｄ[25-(OH)D]值变化及不良反应发生率。结果 观察组的治疗总有效率95％(38/40)明显高于对照组的75％(30/40)(P<0.05),观察组治疗后的中医证候积分低于对照组 (P<0.01),观察组ODI评分、VAS评分均比对照组的评分更低(P <0.001),JOA评分比对照组更高(P <0.001),观察组血清中ALP水平低于对照组(P <0.001),25-(OH)D水平高于对照组(P <0.001)。两组治疗前后的血清Ca水平比较均差异无统计学意义(P >0.05)。观察组骨密度值高于治疗前(P <0.05),对照组则治疗前后无明显差异(P >0.05)。对比两组治疗后骨密度值,也无明显差异(P >0.05)。结论 鹿角为君温肾通络方联合PKP治疗OVCF效果更显著,术后1月患者腰背疼痛减轻更明显、功能恢复更佳、血清中骨代谢指标改善更好。 |

| Objective To investigate the clinical effect of wenshen tongluo decoction with the major medicine antler combined with PKP on OVCF. Methods The study subjects were patients with acute OVCF who received PKP in the Department of Orthopedics of our hospital from January 2022 to December 2022. 80 patients were randomly assigned to two groups by odd-even method: observation group (40 patients) and control group (40 patients). The control group received PKP and routine anti-osteoporosis treatment, and the observation group received wenshen tongluo decoction with the major medicine antler on the basis of the control group. The total effective rate, TCM symptom score, VAS, ODI, JOA, serum calcium (Ca), alkaline phosphatase (ALP) and 25 hydroxyvitamin D[25-(OH)D] values and the incidence of adverse reactions were compared between the two groups after 1 month of treatment. Results The total effective rate in the observation group was significantly higher than control group [95% (38/40) vs 75% (30/40)](P< 0.05), the TCM syndrome score of the observation group after treatment was lower than that of the control group (P<0.01), and the ODI and VAS of the observation group were lower than those of the control group (P< 0.001). JOA was higher than that of control group (P <0.001), serum ALP level in observation group was lower than that of control group (P <0.001), and serum 25-(OH)D level was higher than that of control group (P <0.001). There was no significant difference in serum Ca levels between the two groups after treatment (P >0.05). Bone mineral density (BMD) in observation group was higher than before treatment (P <0.05), but no significant difference was in control group (P >0.05). There was no significant difference in BMD between the two groups after treatment (P >0.05). Conclusion wenshen tongluo decoction with the major medicine antler combined with PKP has more significant effect in the treatment of OVCF, and the patients have more obvious relief of low back pain, better functional recovery and better improvement of serum bone metabolism index 1 month after surgery. |
