      肝性脑病(HE)是严重肝病常见的致死性并发症,氧化应激在其发病中起关键作用。血浊理论是国医大师王新陆教授提出的创新性中医理论。本文发现血浊理论与氧化应激在病理本质、致病过程及临床特征上高度契合,提出“血浊-氧化应激”轴是理解HE病机的关键桥梁。本文认为脾肾亏虚、浊邪内生是HE的发病基础;血浊蕴结,酿生浊毒,上扰清窍是其核心病机。治疗应以“清化血浊”为总纲,贯穿疾病全程,并根据疾病阶段灵活运用健脾化浊、通腑泄浊、解毒开窍等法。本文为中西医结合防治HE提供了新的理论依据与临床思路。

      Hepatic encephalopathy (HE) is a common fatal complication of severe liver disease, and oxidative stress plays a key role in its pathogenesis. The Blood Turbidity Theory is an innovative theory proposed by Professor Wang Xinlu, a master of traditional Chinese medicine. This article identifies a high degree of consistency between the Blood Turbidity Theory and oxidative stress in terms of pathological essence, pathogenic process, and clinical characteristics, and proposes that the “Blood Turbidity-Oxidative Stress” axis serves as a key bridge for understanding the pathogenesis of HE. This article considers that deficiency of the spleen and kidney with endogenous production of turbid pathogens is the pathological basis of HE, while accumulation and binding of blood turbidity generating turbid toxins that disturb the clear orifices upward constitutes its core pathogenesis. Treatment should take “clearing and resolving blood turbidity” as the general principle throughout the entire course of the disease, with flexible application of methods such as strengthening the spleen and resolving turbidity, unblocking the bowels to drain turbidity, and detoxifying and opening the orifices according to the disease stage. This article provides a new theoretical basis and clinical approach for the integrated traditional Chinese and Western medicine prevention and treatment of HE.
